He was predeceased by: his in-laws, Bill and Margaret McKee; and his grandson Caleb Michael Epley. He is survived by: his wife Jayma Cook (McKee); his daughters, Stephanie Jones (Bryan) of Graysville, AL and Shannon Metcalfe (Ryan) of Mt Washington Ky; his siblings, Mark Cook (Brenda) and Kendra Penn (Callie); his sister-in-law Gaye Morris; his nieces and nephews, Mindy Clark, Meridy Cooksey, Aimee Lowrey, Mark Cook Jr., Graham Penn and Jackson Penn; and his pet Buddy Cook. He is also survived by 8 great nieces 2 great nephews, 1 great-great niece and 1 great-great nephew.
