He was predeceased by: his parents, Jack Perkins and Marie Perkins (Riggle); and his granddaughter Alexua Nicole Smothers. He is survived by: his children, Michael Perkins, Coy Smothers, Josh Grutzius (Sacha), Thomas Jophlin, Angie Perkins (Bobby Humphrey) and Lisa McRoy; and his sisters, Darla Drew, Penny Linger (Todd) and Barbie Gulley.
