He was predeceased by: his parents, Norma Haxby and Robert Haxby; and his sister Heather Zielinski. He is survived by: his wife Paula Polizzi Haxby; his son Joe Haxby; his sister Hollis Haxby of Chicago; his brother-in-law Michael Zielinski; and his aunt Jean Fichter (Uncle Bob Fichter). He is also survived by nephews, niece, and cousins.
