She was predeceased by: her parents, Donald A. Cusac and Judith A. Sommers Cusac. She is survived by: her husband Stephen M. Bergquist; her daughters, Kourtney Grossman (Ryan) of Gilbert, AZ and Katelyn Walters (Michael) of East Peoria; her sister Jodi Nimmo of East Peoria; her stepbrother Joseph Cusac of Hanna City; and her grandsons, Maxwell Grossman, Lenox and Porter Walters.
Memorial donations may be made to St. Patrick Church or School in Washington.
