He is survived by: his parents, Hugh E. and Mae Dunn Hazelwood; his spouse Crafton; his children, Jerry (Terri Hazelwood) of Greers Ferry, AR, Linda Diane Levery of Jefferson City, MO and Larry (Dianna Hazelwood) of Washington, IL; his grandchildren, Niki Aberle and Jamie McKean; his great grandchildren, Kody, Jade and Layla; and his siblings, Carrol Hazelwood (Betty) of East Peoria, Bernice Bailey of East Peoria and Dorothy Hunt of Little Rock, AR..
