He is survived by: his child Rehab; his wife Diane; his children, Tia, Tasha and John III; his grandchildren, John IV and Abby; his mother Louise; and his sister Joanne (Mike). He is also survived by many nieces, nephews and extended family members and friends.
In lieu of flowers donations would be appreciated to St. Jude Children's Research Hospital, he felt that no child should have to endure what he has gone through the last four years.
