He is survived by: his wife Kathy Woolsey; his father Herbert Woolsey (Mary); his daughters, Nicole Woolsey, Shannon Tate and Amber (Jake Weischedel); his siblings, Rick Woolsey, Ryan Woolsey (Brenda Dunaway), Raymond Woolsey (Coni) and Rita (Tim Metzger); and his sister-in-law Cindy Woolsey of Patoka. He is also survived by 10 grandchildren; 3 great-grandchildren; numerous nieces, nephews, extended family and friends.
