He is survived by: his wife Diane Mayfield (Eckhardt); his sons, Chad (Shannon) and Matthew Mayfield (Alice); his grandchildren, Brittany Siebert, Brooke Mayfield, Chase Mayfield, Natalie Mayfield and Claire Mayfield; and his siblings, Michael Mayfield, Jeremy Mayfield (Rebecca) and Sheri VonBehren (Allan). He is also survived by many cousins, nieces, nephews and friends.
