He was predeceased by: his parents, Eugene Hoffman and Bernice Hoffman; and his siblings, Mike Hoffman (Sandy), Dan Hoffman (Tina), Joyce Wiltzius (Bob) and Mark Hoffman (Linda).
He is survived by: his wife Phyllis; his children, Jennifer Hodde (Shelby), Jillian Walters (Philip), John (Emily) and Hayley Hoffman (Patrick Sofer); his siblings, Genne Urbanski (Norb), Marty Hoffman (Cheryl), Teri Deckelman (Kurt) and Matt Hoffman (Kelly); and his grandchildren, Emerson, Raegan, Caden, Zachary and Jackson.
In lieu of flowers, memorials to the American Heart Association graciously appreciated.
