He was predeceased by: his father Larry Mendenhall. He is survived by: his mother Barbara; his sisters, Debra Sansom of Decatur and Dawn O'Fallon (John) of Crystal Lake, IL; his niece Paige O'Fallon; and his nephew Sean O'Fallon.
