He is survived by: his nieces, Stacy Thomas, Kayla West and Kelsey Walker; his parents, Donald Larry and Sarah Phelps (Blakemore); his wife Sherry Clevenger; his children, Hannah of Streator and Travis of Streator; and his sister Karen Weldy (Devon) of Wakarusa.
