He was predeceased by: his parents, John Pshak and Mary Pshak (Perhach); his great grandchild Breken; and his brothers, John, William, Robert and Ronald Pshak.
He is survived by: his wife Sandra Shaffer; his children, Julie Smetanko (Michael) of Streator, Brenda Swiskoski (Michael) of Streator and Dr. Thomas Pshak (Trisha) of Denver, Colorado; his grandchildren, Keegan Smetanko, Jessica Swiskoski, Ann Marie Fitzgerald (Logan), Nicholas Swiskoski, Oliver Pshak, Camille Pshak and Heidi Pshak; his great grandchild Maisie Fitzgerald; and his mother-in-law Hazel Fellows.
