He was predeceased by: his parents, Clyde Hert and Bonnie Hert (Ashley). He is survived by: his nephew Ron Crouch (Nate Phelps); his children, Garry Hert (Lori) of Waukesha, WI, Danny Hert (DeShona) of Bloomington and Sherry Hert of Streator; his grandchildren, Josh, Megan, Jessica and Abbey; and his siblings, Larry Hert of Streator, Jerry Hert of Arizona, Willie Hert (Debbie) of Streator, Kay Larson (Stan) of Streator and Bonnie Tyler of Tennessee. He is also survived by 9 great-grandchildren.

Memorials may be made to a charity of the donor's choice or the family of Jack Hert.
