She was predeceased by: her mother Norma Moxley; her stepfather Pete Moxley; her daughter Jennifer; her brother Gari Cook; her nieces, Kayla Cook and Allissa Martin; her great-niece Isabella Cook; and her brother-in-law James Butzen.
She is survived by: her husband Bernard "Ben" Butzen; her stepchildren, Heidi LeGare (Erik Hoffman) of Ottawa and Todd Butzen (Bonnie Ferrarini) of Streator; her granddaughter Bailey LeGare; her siblings, Chris Cook (Barb) of Watkins Glen, NY, Fritz Cook of Manchester, NH, Karl Cook, Hans Cook (Mary) of Streator, Kurt Cook Sr. (Melissa) of Streator, Pete Moxley (Skeeter) of Streator, Bambi Emm (Bob) of Kernan, Shawn Anderson of Havelock, NC and Heidi Cook of Lincoln; and her sisters-in-law, Christine Bateman of Decatur, Rose Hajny of Washington, IL, Cecilia Cleveland (Randy) of Stuart, FL and Nesa Rieuf (Richard) of Marseilles. She is also survived by several nieces and nephews and great nieces and nephews.
Memorials may be made in her name to the Domestic Violence Programs, Drug/Alcohol Program or Children's House Program or a charity of the donors choice.
