He was predeceased by: his parents, Thomas Hornsby and Irene Hornsby (Hart); his daughters, Shelly Hornsby and Valerie; and his siblings, Norma Gaddis, Shirley Sullivan and Thomas Wayne Hornsby.
He is survived by: his spouse Shirley Gass; his daughter Donna Hornsby of Streator; and his sisters, Carol Delheimer (Tom) of Oklahoma, Pam Papenburg (Clarence) of Blackstone and Linda Kinkin of Streator. He is also survived by several nieces and nephews.
Memorials may be made to the Alzheimer's Association, American Cancer Society or the American Heart Association.
