He was predeceased by: his parents, Sherman Forrest and Myrna Forrest (Lee); his sister Linda; and his granddaughter Amber Bixby. He is survived by: his wife Ruth; his children, Tammy Ankebrant (Paul), Diane Combes (Ron), Heather Bell (Bret), Shawn Shugrue (Tiffany) and Melony Taylor (Bob); and his siblings, Julia Lane, Ralph Forrest, Noni Steines, Michael Steines, Valerie Kalemba and Adam Kalemba. He is also survived by 22 grandchildren and 8 great-grandchildren.
In lieu of flowers, the family prefers donations to be made in Rick's honor to St Jude's or Shriners.
