She was predeceased by: her parents, Michael Naccarato and Carmela Petrullo Naccarato; her brother Salvatore Naccarato; her sister-in-law Nancy D'Amaro Naccarato; her brother Michael Naccarato; her daughter-in-law Nancy Sly D'Amaro; and her siblings-in-law, Albert Giardina and Josephine D'Amaro Giardina.

She is survived by: her husband Arthur David D'Amaro; her sons, David (Jennifer), John (Rosemary) and Douglas D'Amaro (Laurie); her grandchildren, Michael (Becky), Michele Williams (Reginald), Steven (Alejandra), Brian and Nathan; her sister Rosemary Jones (Austin); and her sister-in-law Rene Naccarato. She is also survived by nieces and nephews.

In lieu of flowers, donations in Carolyn's memory may be made to St. Jude Children's Research Hospital.
