He was predeceased by: his daughter Mary Jane; and his brothers, Jerry and Robert. He is survived by: his spouse Mary Jane Cassiday of Spring Valley; his father Harry A. Cassiday, Sr.; his brother Wholesale; his daughters, Sarah Gaumond (Peter) of Gaithersburg, MD, Nora Cosgrove of Spring Valley and Maria Drager (Dan) of Medford, WI; his grandson Christopher Gaumond of Gaitherburg, MD; and his significant other Mary Jane Cassiday Cosgrove.
In lieu of flowers, memorials may be directed to a fund that will be established at a later date in Jack's name.
