He was predeceased by: his father John Cundiff (Jay). He is survived by: his mother Rose Cundiff of Sparta; his wife Rena Cundiff of Sparta; his children, John David Cundiff (J. D.), Reka Tabb (Chris) of Clarksville, TN and Becky Ballard (James) of Hopkinsville, KY.; his sister Jaylene Cerny; and his brother-in-law Michael of Anna. He is also survived by three nephews and one niece.
By executive order, face masks are required, and social distancing guidelines must be followed.
Memorials may be given to the Shriner's Hospital.
