He was predeceased by : his parents, Esther F. Forrest (Allgood) and John R. Forrest Sr.; his siblings, John "Jack" R. Forrest Jr., Robert "Bob" C. Forrest and Marjorie "Marge" Homeier (Forrest); and his daughter Diane Davenport (Forrest).

He is survived by : his son Dean E. Forrest (Anita) of Minneapolis; his grandchildren, Jessica P. Davenport, Nicholas D. Davenport of Cedar Lake, Indiana, Andrew "Drew" W. Forrest (Katie) and Nathaniel D. Forrest of Minneapolis, Minnesota; and his great grandchild Hunter. He is also survived by many nieces and nephews.
