He was predeceased by: his parents, Albert Ciosek and Theresa Ciosek. He is survived by: his wife Karen; his daughters, Tara Marie Ciosek, Nicole Ann Ciosek and Kimberly Rose Ciosek; his grandchildren, Jason Jordan Ciosek, Autumn Rose Ciosek and Jaxson Lee Ciosek; and his siblings, Kenneth Ciosek, Anna Simpson (Jerry), Paul Ciosek and Ruth Mahler (Robert). He is also survived by numerous nieces, nephews and cousins.
He will be laid to rest privately at Abraham Lincoln National Cemetery in Elwood. Due to the Covid-19 restrictions maximum occupancy at the church will be 50 guests.
