It is with great sadness that we announce the death of Mary Hazel Phelps (Salem, Illinois), who passed away on June 8, 2018, at the age of 100, leaving to mourn family and friends. Family and friends are welcome to leave their condolences on this memorial page and share them with the family.

She is survived by: her parents, Steve D. and Florence Vaughn (Marshall); her spouse Newton S. Phelps; and her son David Ray Phelps of Salem. She is also survived by 2 granddaughters and several great grandchildren.
