He was predeceased by: his father Lowell Whitehead. He is survived by: his mother Phyllis Whitehead (Jenkins); his girlfriend Linda Dye; his children, Matt Whitehead, Michael Whitehead (Crystal), Chris Whitehead (Jessica), Adam Whitehead (Sarah) and Jamie Whitehead; his sister Diana Bogart; his grandchildren, Jade, Haylen, Oliver, Aliyah, Coy, Cooper, Odell and River; and the mother of his children Donna Whitehead.
It is recommended that guidelines set forth by the Center for Disease Control and Illinois Department of Public Health as they relate to COVID-19 be followed.
