She was predeceased by: her parents, Marvin Smith and Norma Jean Smith-Teel (Gossage). She is survived by: her husband Phil of Rushville; her children, Angie Forman of Morton and Michael Forman of Macomb; her grandchildren, McKenzie, Malachi and Layla Forman; her siblings, Robert Teel of Macomb and Margaret Teel of Macomb; and her pet Cinnamon. She is also survived by several nieces and nephews.
