He was predeceased by: his brother Daniel Chapman; and his grandparents, Arthur, Marilyn Wisniewski and James Perkins. He is survived by: his parents, John Perkins and Gail Chapman; his stepmother Linda Perkins; his daughters, Kaylee and Ember Perkins; his siblings, Kelly Pehler (John) and Jon Perkins (Nancy); and his grandmother Jan Perkins. He is also survived by many nieces and nephews.
