She was predeceased by: her husband Edward Kulak; her parents, Peter Maduzia and Tracy Maduzia (Kopczynski); and her sisters, Bertha Mae and Valeria Przyzycki.
She is survived by: her children, Ron Kulak (Marilyn) and Bernadette Wozniak; her son-in-law Ted Wozniak; her grandchildren, Peter (Delilah Zak), Nicholas (Kelly), Richard, Jason Kulak, Christopher Wozniak (Jennifer) and Kathryn Wozniak (Jake Galuska); and her great grandchild Harrison Kulak.
