He was predeceased by: his father William Hoffman. He is survived by: his mother Kathleen Hoffman (Johnson); his wife Amanda Leslie; his son Joah; and his siblings, Jeff Hoffman (Michelle) and Heidi Larson (John). He is also survived by aunts, uncles, cousins, nieces and nephews.
Visitation will be held on Monday, May 5th 2025 from 4:30 PM to 5:45 PM at the North Main Chapel (2811 N Main St, Rockford, IL 61103). A memorial service will be held on Monday, May 5th 2025 at 6:00 PM at the same location.
In lieu of flowers memorials may be made to the family.
