He was predeceased by: his father Delmar Pontnak; his brother Matt Edward; and his nephew Kirk Daniel Eichholz. He is survived by: his mother Hazel Pontnak (Wagner) of Freeport; his brother Rick Pontnak (Diane) of Freeport; and his nephew Kenneth Eichholz Jr. (Toni) of Lake Somerset.

In lieu of flowers, Terry requests contributions to forwarded to Polo High School Band or St. Marks Lutheran Church in Polo.
