He is survived by: his children, Emma Rae Lavery, Jack Thomas Edward Melchi, Delilah Mica Melchi, Alyssa Marie Melchi, Miah Sharon Melchi and Kelly Melchi; his wife Sarah Melchi (Lavery); his parents, Edward Melchi and Deborah Melchi (Crawford); his sisters, Megan Hazen (Dean), Page Streb (Matthew) and Quinn Murray (Christopher); his father-in-law Ray Lavery; his sisters-in-law, Kelsey Lavery and Kathleen Lavery; and also Kelley Lavery (Poirel). He is also survived by numerous nieces, nephews and cousins and by his Mother-in-law.

In lieu of flowers, monetary contributions to support his wife, Sarah, his children and their families would be appreciated.
