He was predeceased by: his parents, Russell Booker and Thelma Oleta Bulak (Cox); his wife Janice Skowronski; his stepfather Edward Bulak; and his grandson Brandon Booker.

He is survived by: his sons, Keith (Carol) of Pensacola, FL, Jeff (Jan) of Spring Valley and Robert (Lori) of Algonquin; and his brothers, Tim Bulak of Spring Valley and Edward Bulak of Naperville. He is also survived by ten grandchildren, and countless great-grandchildren.
