He was predeceased by: his mother Carol Ann Luecht (Heuermann). He is survived by: his wife Cuc Pham of Peoria; his children, Tien Nieves (Nguyen) of Brimfield, Terra Pham of Peoria, Binh Hoang of Michigan, Brittany Pham of Peoria and Gina Pham of California; and his grandchildren, Kim, Jessica and Harrison.

In lieu of flowers, the family asks that donations be made to the Thien Tam Buddhist Temple.
