He was predeceased by: his parents, Freeman "Dan" Phillips and Marie Phillips (Heichelbech); and his siblings, Danny Phillips and Barb Mousty. He is survived by: his granddaughter Brittney McCarten (Phillips); his wife Sally (Merrill); his sons, Mark and Mike; and his sisters, Ginger Phillips and Sandy Printy. He is also survived by several nieces and nephews.

In lieu of flowers, memorials may be made to EPIC or HISRA in Peoria, IL.
