She is survived by: her parents, Shawn Allen of Forest City and Lori Ann Wertz (Fortna) of Pekin; her fiancé David Capps of Pekin; her daughter Maizie Jean Capps of Pekin; her stepchildren, Gage Boker of Pekin, Jett Daniels of Pekin and Taylor Capps of Pekin; her siblings, Shawna Wertz of Pekin and Chad Fortna (Natosha) of Scottsdale, Arizona; and her grandparents, Jacqueline Fortna of Pekin and Lewie Wertz of Pekin. She is also survived by aunts, uncles and cousins.
In lieu of flowers, memorial contributions may be made to an account for her daughter, Maize, in care of the family.
