He is survived by: his wife Diane Smith; his daughters, Melissa Smith and Angie; his grandsons, Andrew, Jonathan, Bryce, Brant and Caleb; and his siblings, Larry Smith of Orlando, FL, Jon Lotta (Renee) of San Deigo, CA and Trudy Khan of Rockford, IL..

A memorial donation will be made to Friends Forever Shelter at a later date.
