He was predeceased by: his parents, Floyd Cook and Kathryn Cook (Kime); and his brother Allyn Cook. He is survived by: his wife Wilma Potts; his children, Curtis Dale Cook (Laura) of Oregon and Kerrick Douglas Cook (Michelle) of Paris, Illinois; his sister Alice Lauher of Ballwin, Missouri; and his grandchildren, Travis, Kathryn Drew, Rileigh and Charleigh Cook. He is also survived by three great grandchildren, and several nieces.

Memorials may be made to the Vermilion Methodist Church or the Vermilion Volunteer Fire Department.
