He was predeceased by: his grandparents, Steven, Clara Cook (Tabor), William and Dorothy O'Keefe (Urbanc). He is survived by: his parents, Peter Cook and Camilla Cook (O'Keefe); his children, Peter and Kassie Cook; and his siblings, Brian Cook and Lori Cook. He is also survived by numerous aunts, uncles, cousins, nieces, and nephews.
