He was predeceased by: his grandparents, Mike Kuzmich, Ted and Marian Tuleck. He is survived by: his parents, John Kuzmich of Ottawa and Amy Kuzmich (Tuleck); his grandmother Jackie Kuzmich of Naplate; and his brother Kyle Kuzmich.

Due to current State of Illinois guidelines social distancing and face covering will be required.
