He was predeceased by: his parents, Stephen Cook and Clara Cook (Thabor); and his siblings, Frank, Nick, Steve Cook and Mag Nelson.

He is survived by: his wife Carol (Twait); his children, Shelly Kuhn (Steve) of Ottawa, Julie Scott (Kirk) of Wisconsin and Mike Cook (Jane) of Mundelein; his grandchildren, John (Adrienne), Alex (Kate), Leah Kuhn, Shane, Christa, Cara Scott, Matthew and Caitlin Cook; his great grandchild Maya; and his siblings, Joseph (Val) of Ottawa, Pete Cook (Cam) of Ottawa, Rose Hinman (Parke) of South Carolina and Margaret Gunderson (Gary) of North Carolina.
