He was predeceased by: his parents, Benito Ortiz and Juana Ortiz (Rojas); his wife Ruby Ortiz (Biggs); his siblings, Mary Perez (Ortiz), Richard (Dick), Anthony (Tony) and Daniel Ortiz (Danny); his sisters-in-law, Gay Ortiz, Margaret Ortiz and Pat Ortiz; and his nephew Richard Perez (Rick).
He is survived by: his sons, David Jr. Ortiz (Donna) and Jeffrey Ortiz; his grandchildren, Allyssa Sereno (Mike), Alexandra DuBois (Kent) and Michael Ortiz (Tania); his great grandchildren, Olivia, Bailey, Evelyn, Alaina Sereno, Kaleb and Gianna DuBois; his brother Dr. Phil Ortiz; and his siblings-in-law, Raymond Perez and Judy Ortiz. He is also survived by family members and friends.
