He is survived by: his father Edward Polaski; his wife Joan; his daughter Julie; his siblings, Rick (Ellen) and Christy; and his nieces and nephews, Nora, Quinn, Duncan, Tori, Nina, Delaney, Jennifer, Rebecca, Matthew and Ryan.

In lieu of flowers, donations may be made to the American Diabetes Foundation.
