He was predeceased by: his sibling Sandra Leinerth (Ron). He is survived by: his spouse Nancy; his children, Tiffany, Connie, Raymond Kozak (Robyn), Steven Kozak (Renee) and Kevin Kozak; his grandchildren, Devin, Jacob, Ethan and Mason; and his nieces and nephews, Pam Addleman, Jeff Leinerth (Cheryl), Ron Leinerth (Sheilla) and Dawn Reynolds (Jason).
