He was predeceased by: his parents, Gary L. Smith and Donna D. Smith. He is survived by: his daughters, Angela Murphy (John) of Hoopeston, Heather Dean (Brian) of Defiance, OH, Brittany Hopkins of Penfield, IL, Brooke Taylor of Danville and Cara Smith of Danville; and his sisters, Elaine Kinney (Chris) and Gayle Marett (Jim). He is also survived by twelve grandchildren.
In lieu of flowers, memorials in Jay's name may be made to the Vermilion County Animal Shelter.
