He was predeceased by: his wife Lenore; and his siblings, Billy Formella and Shirley Noffke. He is survived by: his children, Frank Formella (Charlane), Leona Plocharczyk (David), Jerry Formella (Jill Jordan), Roberta Crane (late Ken) and Tina Keyser (Kenn); his grandchildren, Dana Plocharczyk (Ryan Strachan), Valerie Gallagher (Dan), David Plocharczyk, Keith Plocharczyk, Paige Crane and Melanie Crane; his great grandchildren, Aryanna, Mackenzie, Emily and Olivia; and his sibling Sandy Durante. He is also survived by many nieces and nephews.
In lieu of flowers memorials to Arthritis Foundation would be appreciated.
