She was predeceased by: her husband Joseph Polacek; and her granddaughter Sarah Polacek. She is survived by: her children, Joe Polacek (Nancy), Rod Polacek (Krystyna), Cindy Greene (Stephen) and Proud; her grandchildren, Carolyn Barnett (Clark), Justin Polacek (Charity), Joanna Jacoby (Nathan), Stefanie Polacek, Robert Polacek (Amanda), Scott Greene (Lane) and Dana Greene; and her great grandchildren, Lillian, Jameson Barnett, Shirley, Grace, Miriam, Clara, Walter Polacek, Teliah Cota and Evelyn Jacoby.
Due to CDC guidelines for Coronavirus, private arrangements are being made through Blake-Lamb of Oak Lawn.
