She is survived by: her husband Mitchell Formato; her son James Formato (Victoria); her grandchildren, James, Gina and Brianna; her great grandchildren, Gavin and Gigi; her brother Joseph Donofrio (Dede); and her nieces and nephews, Joseph Nicolette (Patty), Angela (John), Nicholas, Mariah (Nick), Michael (Ariana), Anthony (Jen), Frank (April), Kara, Andrea, Joelie, Tabitha (Michael), Christy (Doug) and Ryan (Jordan).
