He was predeceased by: his parents, Dominick Kusiak and Julia Kusiak. He is survived by: his wife Susan Janice Blaszynski; his sons, Michael Kusiak (Kim) and Daniel Kusiak (Jennifer); his grandchildren, Brian, Sabryna and Amanda; and his siblings, Paul Kusiak (Elnore), Anne Kusiak and Mary Kusiak. He is also survived by two great-grandchildren.
