He was predeceased by: his parents, William and Mary V. Henzlik; and his siblings, Patricia Bennett, Rev. William C. and Dr. Raymond E. Henzlik.
He is survived by: his wife Barbara Ann Boller; his children, Jennifer Durnas (Jason) and William Henzlik; his grandchildren, Evan and Lucy Durnas; and his siblings, James Busse and Kathleen Anderson (John). He is also survived by many nieces and nephews.
