He is survived by: his wife Sharon Ann Plank; his daughter Melissa Safarik (Ron) of Austin; his stepdaughter Amy Myers of Omaha; his brother Jeff Fulks (Kathy) of Hudson; his grandchildren, Sydney and Natalie Safarik; his stepgrandson Jared Myers; and his nephews, Kevin Fulks (Brett) of Mahomet and Brian Fulks (Lacey) of Normal.
