He was predeceased by: his parents, Roy Cook and Eva Cook (Miller); and his sister Linda Lou. He is survived by: his wife Jean Paris; his sons, Kevin Cook (Katie) of Normal and Brian Cook (Lisa) of Normal; his grandchildren, Melissa Ulrich (Darren), Jared Cook (Amanda), Cody Cook, Lee Cook and Janene Cook; his great grandchildren, Eva Ulrich, Levi Ulrich, Emma Ulrich, Lukas Ulrich and Liam Ulrich; and his siblings, Jerry Cook (Louanne) of Streator, IL and Judy Kopriva (Larry) of Placentia, CA..
