He was predeceased by: his mother Geraldine. He is survived by: his wife Dawn; his daughters, Samantha Dybas (Nick), Kylie Kraft (Adam) and Natalie Cousins; his father Thomas James Cousins; his brothers, Mike Cousins and Steve (Kristin); and his cousin Ken (Dana). He is also survived by Cousins.

In lieu of flowers, donations can be made to the Neuroendocrine Tumor Research Foundation.
